Support work - Community Care Workers
Our care is provided to those in receipt of Home Care Packages NDIS. If you receive a different funding body or are privately funded please reach out to see how we can help you.
Community Nursing
Our nurses provide hands on nursing care, assessment, care planning, wound care, medication management and Telehealth consultations. If we are unable to provide the care you require we will know who can and refer you to the most appropriate health care professional.
Health and Care Coordination
Our team will support you in building a healthier you. No matter what your health needs are, having a team support you will keep you on the path to meeting them. We work together to connect you with the services you need. We will assist you to navigate the healthcare system and refer you to other providers as needed.
We advocate for you to ensure the best possible outcome to maintain your health and happiness at home.
Support work - Community Care Workers
We provide hands on care that is flexible and tailored to you.
We work not just with you but with other members of our community to build a network of people working together for you. We also include your significant others in your life who are important to your health and wellbeing.
